You can open and download useful forms and other documentation from this page.
To do so, left-click on the link. The document will usually open in a separate window, using Adobe Acrobat or other reader [viewer] software. At the top left of the viewer, click File, Save As... and save it to your own computer. You can also print most documents, using File, Print. Alternately, you can right-click and save immediately.

| Educational Visit Information and Consent Form (.pdf) | 450kb | Visits, tours and other offsite events may require this form. SYO's conductor has been assured by the County Outdoor Education Officer that one trawl for this information per year is deemed sufficient; hopefully this extends to other ensembles too! However, if you do change your contact phone numbers mid-year, or there is a new medical condition, or anything else that should be known about, please remember to inform your ensemble director. |

Most of the downloadable documents on this site are in .pdf (portable document) format. If you do not already have a .pdf reader on your PC, you may like to download Foxit or similar free reader. These are legal products, and much smaller than Adobe Acrobat Reader.
Any MS Word files on this page will be saved to the older .doc format rather than Word 2007's .docx.
